Encounter 'ERROR: Index was outside the bounds of the array.(Row = 1)' when uploading Learning records

Symptoms:

The error occurs when trying to upload learning records using CSV format at Learning > Employee Learning > Learning Data > Learning Record > Upload Records

Possible cause(s):

The file being uploaded is an incorrect file or contain invalid values (did not follow the values available in system) 

Suggested workaround / Solution(s):

Enter the correct value shown in the system and the total column in the excel file must be the same
