1. Administrator to access Menu > Frontier Tools > Form Designer > New Form Template.
- Select form type as Evaluation Form
- Enter the necessary information such as Name, Description etc
- Tick "Is PD Form"
- Ensure Status is set as Active
2. Once created, click on the second icon for the newly created form.
3. Click New Section > Key in required Details > Select Section Type as Course Survey > Save.
4. Click New Questions for Course Survey > key in the questions, select question type > save. Repeat this step if multiple questions is required for the survey.
5. Repeat Step 3 and 4 for Section Type : Pre-Course Briefing, Post-Course Briefing and Target Achievement

It is mandatory to create for the 4 Section types, that is Course Survey, Pre-Course Briefing, Post-Course Briefing and Target Achievement in order for the evaluation form to work
6. Once the form is created, the administrator can assign it to a particular course or the whole entity.
- To assign to a course, go to Course Directory > Select Course > Under Evaluation > Change the form to the newly created form.
- To assign to whole entity, go to Learning Options > Evaluation > Change form to the newly created form.
